Minimum Qualifications
The Licensed Practical Nurse (LPN) provides skilled nursing care to clients in their homes under the supervision of a Registered Nurse. This role is responsible for implementing physician-ordered care, monitoring client conditions, administering medications, and ensuring accurate clinical documentation in compliance with Medicaid, payor, and agency requirements.
Primary Responsibilities
Provide skilled nursing care in accordance with physician orders and care plans

Qualifications

Strong clinical judgment and communication skills
Ability to pass background check, drug screen, and health requirements

The Certified Nursing Assistant (CNA) provides direct, hands-on care to clients in their homes under the supervision of a Registered Nurse or Licensed Practical Nurse. This role supports clients with activities of daily living while observing, reporting, and documenting changes in condition as outlined in the care plan.
Primary Responsibilities
Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and feeding
Support safe mobility, transfers, positioning, and fall prevention
Take and record vital signs as permitted by scope and care plan
Observe and report changes in client condition to nursing staff
Assist with prescribed exercises and range-of-motion activities
Provide comfort, companionship, and emotional support
Maintain a clean and safe care environment
Accurately document care provided in accordance with agency policy
Follow infection control, safety, and confidentiality standards at all times

We are seeking a compassionate, detail-oriented PRN Registered Nurse (RN) to support our skilled nursing operations. This role is primarily responsible for client intake assessments, development of skilled nursing care plans, routine reassessments, clinical documentation, and care coordination in accordance with physician orders, Medicaid requirements, and agency policies.
This position is ideal for an RN who values autonomy, flexibility, and high standards of clinical documentation.
Primary Responsibilities
Conduct comprehensive skilled nursing intake assessments in the home
Develop, implement, and update individualized Plans of Care based on physician orders and clinical findings
Complete revisits, reassessments, and recertifications as required by payors
Document all assessments, visits, and care activities accurately and timely in the agency’s documentation system
Perform medication reconciliation and clinical evaluations
Identify changes in patient condition and promptly communicate with physicians and agency leadership
Ensure all documentation meets Medicaid, Medicare, and payer compliance standards
Participate in care coordination with caregivers, families, physicians, and internal team members
Adhere to all agency policies, state regulations, and professional nursing standards
Required Qualifications
Active, unrestricted Registered Nurse (RN) license in the state of practice
Minimum 1–2 years of clinical nursing experience (home health experience preferred)
Strong knowledge of skilled nursing assessments, care plans, and documentation
Familiarity with Medicaid and insurance documentation requirements
Ability to work independently and manage time effectively

Preferred Qualifications
Home health or skilled nursing experience
